Flag Certificate This certificate comes standard with any flag that has been flown over the United States Navy Memorial. Additional certificates can be ordered here, either as a re placement for one you have already received, or if multiple certificates are desired for one flag. Please complete and submit the form to ensure that your certificate reflects the following: In the Requested Date field, enter the date your flag was originally flown. Whether the flag is being Presented to, or Presented in Memory of the honoree. The complete name of the honoree. Rate/rank is optional. An optional brief personal inscription. It is recommended to keep your inscription between 1-2 sentences of moderate length to ensure legibility on the printed document. You must specify whether the flag flown was a U.S. Flag, USN, USMC, or USCG Ensign. The information typed here will be reprinted exactly how it is entered.
