fluoxetine necklace | sterling silver Fluoxetine changed how depression is treated. Not because it worked better than older drugs, but because it was safe enough for a family physician to prescribe alone. Approved by the FDA in 1987 as Prozac, it moved psychiatric care from specialist consultation into primary care. The Science Behind Fluoxetine Fluoxetine is a selective serotonin reuptake inhibitor (SSRI) that blocks the serotonin transporter (SERT), the protein that pumps serotonin back into the presynaptic neuron after release. By blocking reuptake, fluoxetine increases serotonin concentration in the synapse and prolongs its signaling effect. Developed by Bryan Molloy and David Wong at Eli Lilly in the 1970s through a systematic search for compounds that selectively affected serotonin without the broad receptor activity of older tricyclic antidepressants. The clinical lag (the 2-4 week delay before antidepressant effects appear despite immediate SERT blockade) reflects downstream neuroadaptations, including changes in serotonin autoreceptor sensitivity and hippocampal neurogenesis. Psychiatrists, psychopharmacologists, depression researchers, and people who take Prozac. It is the medication made visible. What does fluoxetine do that older antidepressants didn't? Fluoxetine was safer. Older tricyclic antidepressants had broad activity across many receptor types, creating side effects that made them risky to prescribe at high doses. Fluoxetine's selectivity for serotonin reuptake meant a larger therapeutic window and a side effect profile safe enough for general practice.
