What is Lactose Malabsorption? Lactose malabsorption means that you are not able to effectively digest and absorb the milk sugar lactose. The sugar lactose needs to be broken down by an enzyme (lactase) into two smaller molecules (monosaccharides) in order for it be absorbed across the wall of the small intestine. If there is a deficiency in this enzyme and lactose cannot be broken down, it travels to the large intestine. In the large intestine, normal colonic bacteria metabolise the lactose.
