NEEDS ASSESSMENT & INDIVIDUAL SERVICE PLANNING Interviewing a person’s past and the individuals who represent their core support group can reveal preferences, gifts, strengths, and talents. Assessing needs and service planning is a process and ongoing dialogue with individuals you support and their care teams. In this course, you will learn that each resident has unique capabilities and care needs.
