Traction splinting restores femur alignment, reduces the thigh compartment volume, and relieves muscle spasm, all of which limit ongoing hemorrhage and dramatically reduce pain. However, traditional traction splints (Sager, Hare, Thomas) extend 18–24 inches beyond the patient's foot — making them unsuitable for helicopter transport, vehicle extrication, and litter evacuation in confined spaces. The Slishman Traction Splint solves this problem with a fundamentally different traction mechanism. Rather than anchoring at the foot and pulling distally, the STS applies force at the patient's hip using a proximal pulley system. Telescoping aluminum poles extend from hip toward the ankle, creating a pushing force against the distal strap rather than a pulling force from the foot. The result is a traction splint that remains anatomically contained between the patient's hip and ankle — never extending past the foot — allowing safe use in aircraft, vehicles, and narrow egress paths without modification. The STS is applicable in lower leg injury or foot amputation scenarios where traditional traction splints are contraindicated. The distal strap can be positioned proximal to the calf, still providing traction to the femur while leaving the distal injury accessible for treatment.
