Teams That Care brings staff together to talk about what makes a successful team, and the improvements they can make to become more effective. As health & social care integration brings different services to work more closely together, new working teams are evolving and must learn to work together effectively. Teams that Care will help these teams think about how to coordinate their approach, working together to provide effective person-centred care.
