POTLUCK PARTICIPANTS: please complete this form no later than November 17th. If we are not able to gather enough balanced potluck dishes, we will need to cancel the potluck portion entirely. An email will be sent to all attendees if this happens by Thursday 11/20 evening 6:00 PM. Tickets are final sale and non-refundable, unless we cancel the event ourselves.
