UrgoStart is a sterile, soft-adherent absorbent foam wound dressing containing Urgo Medical's TLC-NOSF Healing Matrix, designed for the management of exuding chronic wounds at risk of delayed healing. It combines a micro-adherent TLC-NOSF wound-contact layer with an absorbent polyurethane foam pad and semi-permeable backing. The dressing helps manage wound exudate while maintaining a moist wound environment and reducing excess matrix metalloproteinase (MMP) activity associated with delayed wound healing. UrgoStart is intended for use from the appearance of granulation tissue through to complete wound healing. TLC-NOSF combines Technology Lipido-Colloid with Nano-OligoSaccharide Factor (NOSF). Chronic wounds can contain excessive levels of matrix metalloproteinases, enzymes which can degrade proteins required for normal tissue repair. The TLC-NOSF matrix helps reduce excess MMP activity and supports a wound environment more favourable to healing. The lipido-colloid wound-contact layer also helps: Maintain a moist wound environment Minimise adherence to the wound bed Support gentle dressing removal Reduce trauma to newly formed tissue Absorbent foam layer: The dressing incorporates a polyurethane foam pad which helps: Absorb wound exudate Drain fluid away from the wound surface Reduce excessive moisture at the wound margins Help protect the surrounding skin from maceration The semi-permeable outer backing allows moisture vapour transmission while helping protect the dressing. Suitable wound types include: UrgoStart is indicated for exuding chronic wounds including: Venous leg ulcers Other leg ulcers Diabetic foot ulcers Pressure ulcers / pressure injuries It is intended for use from the appearance of granulation tissue until complete healing. The underlying cause of the wound should always be managed appropriately. For example: Appropriate compression therapy may be required for venous leg ulcers Off-loading may be required for diabetic foot ulcers Pressure redistribution is required for pressure injuries How to use: UrgoStart should be applied by, or according to the advice of, an appropriately trained healthcare professional. Wound preparation: Clean the wound according to the usual wound-care protocol. If an antiseptic has been used, rinse the wound carefully with normal saline before applying UrgoStart. Carefully dry the surrounding skin. Select the appropriate dressing size. UrgoStart can be cut using sterile scissors to fit the shape and size of the wound where necessary. Application – standard dressing: Remove the protective tabs. Apply the micro-adherent side directly to the wound. Ensure the wound surface is appropriately covered. Secure the dressing with a suitable: Retention bandage Tape Other appropriate fixation system Apply compression therapy over the dressing where clinically prescribed. UrgoStart does not have an adhesive border, so appropriate fixation is required. Application – Heel dressing: The Heel version is specially shaped for application around the heel. Remove the protective tab. Position the arrow-shaped section towards the front of the foot. Position the heel centrally on the micro-adherent side of the dressing. Apply the rear section over the Achilles tendon. Remove the side tabs. Apply the side portions carefully around either side of the foot. Secure with an appropriate bandage or fixation system. Dressing changes: UrgoStart should generally be changed: Every 2–4 days on average Depending on: Exudate volume Wound condition Condition of the surrounding skin Clinical progress The dressing may remain in place for up to 7 days where clinically appropriate. Change sooner if: The dressing becomes saturated Exudate leaks through the dressing The dressing becomes displaced The wound requires reassessment Signs of infection or deterioration occur The manufacturer recommends a treatment duration of at least 8 weeks where clinically appropriate. Important information: Do not use UrgoStart on: Malignant / cancerous wounds Fistula wounds associated with a deep abscess Patients with known hypersensitivity to the dressing or its components If the wound shows signs of significant bacterial colonisation or infection, the bacterial component should be appropriately managed before starting UrgoStart. UrgoStart does not replace appropriate treatment of wound infection. In an atypical ulcer with: Induration Excessive localised granulation Unexpected deterioration the wound should be appropriately assessed before beginning treatment to exclude malignancy or another underlying pathology. Epidermolysis bullosa and uncomplicated acute wounds: Because of insufficient clinical data, the manufacturer does not recommend UrgoStart for uncomplicated acute wounds or wounds associated with epidermolysis bullosa. Handling precaution: The micro-adherent lipido-colloid layer may stick to latex surgical gloves. Avoid touching the coated wound-contact surface directly where possible, or use sterile forceps/tongs. Possible discomfort: Stinging or painful sensations have occasionally been reported, particularly when treatment is first started. If significant or persistent discomfort occurs, the wound and treatment should be reassessed. Hyperbaric oxygen therapy: UrgoStart contains a lipido-colloid component. It must not be used during hyperbaric oxygen chamber therapy without an oxygen mask because of combustion risk. Specialist protocols must be followed if hyperbaric oxygen treatment is required.
